All the characters have this sort of "duality" aspect to them.
Arcueid normally acts like an airhead, but in serious situations, she becomes really scary. No one needs to mention Ciel or Kohaku. Hisui acts emotionless, but she's actually capable of a very wide range of emotions.
And then there's Akiha. At first, she always seems like this perfect, proper rich girl. It's only once things start getting really weird (or if Shiki says something REALLY stupid) that she shows just how much of an ill-tempered tsundere tomboy she is.

Akiha actually has a pretty tough facade. It's supposed to be a bit of a shock when she shows her "less proper" side.

You can see it better in pic related. The far left expression is her worried/embarrassed look. To me, it's too subtle, the original tsukihime sprites may have been fairly crude, but they were nicely emotive.

Like I said, it was probably supposed to be more subtle than it actually was. Out of all the characters, Akiha is the one who hides her true self the most. She's the most restrained out of all of them.
I'm sure she has far more emotive sprites. Those are only three. And there's actually no guarantee that those will even be her sprites. For starters, they're clearly just roughs.
