Well, if you're not counting the P3 movies as a good adaptation since they're not a series, I guess Shingeki no Bahamut is the only other good adaptation I've seen that is based on a game.
We've got Gunslinger Stratos coming up this season, and God Eater soon as well, so perhaps that number will go up.

It's definitely tremendously wacky cause one of the Main Antagonists is Hitler equipped with the spear of Longinus and with the "Crawling Chaos" as Persona alongside futuristic Nazi's in Mech's seekinto fulfill the Mayan Prophecy which involves an Alien spaceship.
