Seems like it's a modified standing whiteboard frame, put on some homemade not-so-safe wooden suspension. And it is a brush settings thing. size does not matter since you can just scale the brush down, and you can set how pressure sensetive the brush is in the brush settings. Finding/making a brush that feels really good for doing linework is pretty hard imo.
