Hiroshima was ruled by a Main family that controlled the vigilante squad. The branch families are offshoots of the main family. The city was essentially ruled by a series of clans, with a single ruling clan presiding over them all. At some point the families had a big civil war that essentially destroyed most of them. The main family essentially lost all authority, and remained 'main' in name only. The pirate girl sized control of her branch family and took over the whole city while everyone else was too weak and is now the big cheese of Hiroshima.
The Pirates fought with the Demons at some point, intending to conquer and occupy them. Recognizing that they couldn't win, the leader of the Demons made a secret deal with the Pirates. The Demons themselves were disbanded, and the Rose girl was left behind to collect the taxes that would pay the exorbitant tribute fees to the Pirates, in exchange for their independence.
That's the money that bamboo girl was talking about near the end of the episode. They used the money from the Demon's to build that ship.

The stones are attracted by powerful emotions. Those emotions are what let people level up, what let Hibiki become a 2 and caused a stone to fall from the sky immediately after.
The stones that fall during the rock concert are meaningless, because they aren't brought down by strong feelings of people trying to get stronger, but strong feelings of people rocking out to good music. The stones themselves are irrelevant outside of being rocketfuel, anybody with the drive to become a best will eventually become one, and they'll eventually have a stone come to them, attracted by the motivation that drives them to improve.

What I'm getting from the relative powerlevels here is that Pirate is the bestest best so far. Sort of like how Geisha and Rocker couldn't even touch Boxer, nobody seems to be a match for Pirate.
