As well as Accelerator basically blowing a Sister apart and that time where Accelerator killed an arms dealer who was trying to sell a young girl he routinely tortured (naked, tied) to accel. Of course there's the rest of the horrible things from AC too. And not to forget the magic side with Marian Slingeneyer who turned any fights in horrifying bloodbaths by fighting purely by modifying the bodies of her enemies in terrible ways (like changing some poor guys hands into an open faucet through which his blood drained away. NT4 was really damn bad).

The main problem with that is probably the age of most main characters. After all, a shit ton of them are not adults yet.
Not to say that it's never mentioned in the novels though, like Mugino talking about Shiage and Takitsubo, or during the torture of Brunhild Eiktobel where it's explicitly stated that they didn't sexually abuse her since they didn't want to break her (not to say anything about what they did do to her).
For how inconspicuous this series is it has rather bizarre amounts of torture, mental manipulation, body horror and other crazy shit going on.
