Subtitles on DVDs and blu-rays are stored as images, so you have to have a program recognize the characters if you want to make actual text softsubs out of them. Obviously such programs are far from perfect and very prone to error.

Pretty much all of them.
I'm at a dreadful stage of learning the language where I can tell when they're pulling the subs out of their ass, but can't quite watch raw just yet.
So many subs include needles cursing and joke "localizations".
One of the worst examples was a sub of Chuunibyou I watched recently where Dekomori's desu was eight times out of ten joke-translated as death.
