Ivanovich, one of the heads of the Three Clans true form is a giant ram like demon (think Baphomet) that is all rotting and kind of falling apart, symbolizing his evil nature and his immense age.
Various other vampires true forms have been shown, but not many of them are easy to explain simply because their personalities aren't that defined (just a random villain here or there that wasn't that important by themself) or it is difficult to define because of the form itself. Vera, Mina's sorta mother/older sister advisor has a mist like form which is difficult to explain.
Alphonse, Mina's spymaster, has a true form that looks like a crystal bird creature, which is also difficult to define.
