They could have made Kisumi be Sosuke's friend and reach and agreement with him to keep his secret from Rin to avoid the tears.
It would have given Sosuke another good old friend, making his character less Rin-centered.
It would have created some cheap drama from Kisumi worrying about it and confronting Sosuke about him needing to tell Rin before the big relay.

The injury was exaggerated for drama, they can exaggerate the recovery for a happy ending. It wouldn't be the worst writing this series has seen, either. His character arc was about finding the power of friendship and the injury was just the catalyst, so recovering wouldn't erase any of that.
And it would make him happy. Rin too, and we know the staff loves Rin.

Well, after the show ended, I was one of those people asking 'what the fuck happened to Sousuke', but after giving it much thought, it is much better to leave it all open because seeing him get his shoulder heal would probably piss me off more than the actual injury did.
That being said, I can now imagine Sousuke's parents forcing him to become engaged to a rich US business woman to save the family from bankruptcy.
