They have a way healthier diet and hence an incredibly low obesity incidence rate of I think 2-3%. 1. Calorie restriction. It's been shown you have a 400% reduction in heart disease if your GRANDFATHER was on a calorie reduced diet. This is thanks to genetic switching.
2. Diet. The Japanese diet is very high in fats, and very low in processed sugars. It's considered unmanly to even like sweets and it's considered piggish for girls to eat them. It's true they're popular but if you saw the serving sizes compared to the American ones you'd be shocked. Some people point out they have a diet with high rice, and that's true, but rice is not at all bad for you compared to simple carbs. Brown rice is obviously better though.
3. Location. The air quality is extremely improved compared to a continent.
4. Exercise. Everyone in Japan walks, and they walk constantly. They walk every day to and from school or work. The importance of this cannot be overstated.
5. Genetics. They're small people. Small people tend to live longer just because of a more stable cardiovascular system.
So if you add up all these cardiovascular advantages, a portion of their population killing themselves through smoking isn't enough to keep them from having a longer expectancy than everyone else. Smoking still kills Japanese people though so don't think it doesn't. They just have so many other advantages to make up for it.

Actually several smokers I know were able to quit using e-cigs. One of them in fact is my grandmother who smoked for over 40 years, she tried pretty much everything else like the patches and pills and such, but the electronic cigarettes were the only thing that actually stuck. The problem is that not only do the have a higher life experience than any other country, but they also have lower rates of people getting lung cancer. My grandparents did smoke heavily for quite a few decades up until their deaths at around 75. When my grandmother got sick it was unfortunately her past smoking that prevented her from making a recovery. My grandpa did develop lung cancer and looked incredibly frail by the time he died. But yes people with healthy bodies and good genes can live for quite a while when smoking heavily. Unfortunately it usually isn't a pretty site near the end.

Smoking meka your cells multiply faster but becouse of this they "skip" some phases, more specifically the "verifing" phases, making the odds of creating sick cells higher than persons that don't smoke in any part of the body, so in the end, if you smoke, you may never have a lung cancer but contract pancreatic cancer as a side effect of quick cell replication. You also rise the chances of tumors and your breath gonna stink like shit and your teeth gonna rot
