As far as FB goes, I don't really know because his circumstances weren't covered in the VN. He summarized his situation as "stuff happened". It was rather underwhelming to me. It is entirely possible that he lacks autonomy too, in which case the blame passes entirely to SERN.
And again you underestimate the consequences that conditioning can have on a person. It does remove all agency when they are incapable of taking independent action by the way their thoughts have been molded. Moeka very clearly did not want to kill anyone (do you remember how she was shivering and muttering to herself?), and was simply following orders that she cannot conceive of disobeying.
